The following legal documents have been provided to assist you in receiving the various services that the High Commission performs on behalf of the Government of the Republic of Trinidad and Tobago. These forms can be printed, completed by hand and mailed to the High Commission or brought in person with the appropriate fee and documentation for processing.
Birth Certificate Form
A birth certificate is a legal document needed to establish legal identity and will be used throughout your life to obtain health services, a passport, old age pension and many other beneficial services. This form is used by individuals to request proof (certificates, certified copies) of a birth occurring. Persons born in Trinidad and Tobago are entitled to a free first issue of the recently introduced computerized birth certificate issued by the Registrar General's Department. This is an important requirement when the machine-readable passport is introduced to nationals living overseas.

Death Certificate Form
Death registrations are a permanent, legal record of the death of an individual. This form is used by individuals to request proof of registration (certificates, certified copies) of death occurring in Trinidad and Tobago.

Divorce Certificate Form
This form is used by individuals to request proof (a duplicate of the divorce certificate) of a divorce order occurring in Trinidad and Tobago.

Marriage Certificate Form
This form is used by individuals to request proof of registration (certificates, certified copies) of marriages occurring in Trinidad and Tobago.

(Nationals living in Nigeria may also request these forms personally by asking family members, relatives or friends to obtain same on their behalf. A Letter Authorizing that person is required. With this approach, it is important that you request an 'Apostille' stamp be issued, thereby authenticating this document.)
Restoration of Citizenship
This form is used by individuals to renew their Trinidad and Tobago citizenship status. Medical Forms P&I 40 and P&I 40A must also be completed.
